Fellowship in Geriatric Medicine

1. MD or DO degree from an accredited medical school
2. Completion of an ACGME-accredited Internal Medicine or Family Medicine residency
3. Board-eligible or board-certified in Internal Medicine or Family Medicine
4. Curriculum Vitae and personal statement
5. Letters of recommendation (including residency program director letter)
6. USMLE or COMLEX examination scores
7. Eligibility for medical licensure in Minnesota
8. ECFMG certification for international medical graduates (if applicable)
