Eligibility Criteria

Hold an MD, DO, or equivalent medical degree.
Complete an accredited Cardiovascular Disease Fellowship before the fellowship start date.
Meet COCATS training requirements, including Level I certification during general cardiology fellowship.
Be eligible for a full Illinois medical license.
International applicants must possess a valid ECFMG Certificate and appropriate visa documentation.
Apply through the Electronic Residency Application Service (ERAS).
Submit a completed ERAS application.
Submit a recent photograph.
Submit a current Curriculum Vitae (CV).
Submit a Personal Statement (maximum one page).
Submit USMLE Step I, II, and III transcripts.
Submit ABIM Board Certificate (or documentation of board eligibility).
Submit ACLS and BLS certification copies.
Submit three letters of recommendation, including:
One from the Cardiovascular Disease Fellowship Program Director.
Two from cardiologists familiar with the applicant's work.
Participate in interviews if shortlisted.
