Fellowship Course in Sleep Medicine

1. MD or DO degree from an accredited medical school
2. Completion of an ACGME-accredited residency in an eligible specialty
3. Board-eligible or board-certified in the primary specialty
4. Curriculum Vitae and personal statement
5. Letters of recommendation (including residency program director letter)
6. USMLE or COMLEX examination scores
7. Eligibility for medical licensure in Minnesota
8. ECFMG certification for international medical graduates (if applicable)
