Overview
The Advanced Heart Failure and Transplant Cardiology Fellowship at Johns Hopkins University School of Medicine (JHUSOM) is a one-year, ACGME-accredited fellowship designed to provide specialized training in the management of patients with advanced heart failure and those undergoing heart transplantation. Fellows receive comprehensive clinical experience in advanced heart failure therapies, ventricular assist devices (VADs), mechanical circulatory support, pulmonary hypertension, and pre- and post-heart transplant care. The program offers multidisciplinary training alongside internationally recognized faculty, with opportunities to participate in clinical research, academic teaching, and quality improvement initiatives, preparing graduates for leadership roles in advanced heart failure, transplant cardiology, and academic cardiovascular medicine.

Eligibility
- Must possess an MD, DO, or equivalent medical degree.
- Must have completed an ACGME-accredited Cardiovascular Disease Fellowship before starting the program.
- International Medical Graduates must have completed an ACGME-approved Internal Medicine residency in the U.S.
- Applicants from non-ACGME cardiovascular programs must have at least three years of cardiovascular disease training.
- Only J-1 visas are sponsored for this fellowship.

Admission Process:
Step 1: Submit Your Application via ERAS
- Applications are accepted only through the Electronic Residency Application Service (ERAS).
- Applications are reviewed after they become available in July.
Step 2: Submit Required Documents
Upload the following through ERAS:
- ERAS application
- Official Medical School Transcript
- Curriculum Vitae (CV)
- USMLE transcripts (all steps)
- Personal Statement
- Three Letters of Recommendation, including one from your Program Director (ERAS allows up to five letters).
Step 3: Interview Process
- Applicants selected for interviews are notified in September/October.
- Interviews are conducted during September and/or October and are held virtually.
Step 4: NRMP Match
- The fellowship participates in the National Resident Matching Program (NRMP).
- Applicants and the program submit their rank order lists through the Match.
- Rank Order List Deadline: Mid-November.
- Match Day: Early December.
Step 5: Fellowship Commencement
- Successful applicants complete credentialing and onboarding requirements before beginning the fellowship in July of the academic year.
