Fellowship in Transplant Cardiology

1. MD or DO degree from an accredited medical school
2. Completion of an ACGME-accredited Cardiovascular Disease fellowship
3. Board-eligible or board-certified in Cardiovascular Disease
4. Valid medical license or eligibility for licensure in Minnesota
5. Curriculum Vitae and personal statement
6. Letters of recommendation (including fellowship program director letter)
7. Demonstrated interest or experience in advanced heart failure or transplant cardiology
8. ECFMG certification for international medical graduates (if applicable)
