- MD, DO, or equivalent medical degree.
- Completion of the required residency and subspecialty training.
- Board eligible or board certified in the primary specialty.
- Eligible for Illinois medical licensure.
- Submission of a completed application and supporting documents.
- Successful completion of interviews and credentialing requirements.
