Overview
The Hospice and Palliative Medicine Fellowship at the University of Illinois College of Medicine – Rockford is a one-year, ACGME-accredited clinical fellowship that provides comprehensive training in the care of patients with serious, chronic, and life-limiting illnesses. The program focuses on symptom management, pain control, communication, advance care planning, hospice care, and interdisciplinary teamwork across inpatient, outpatient, hospice, and home-based settings. Fellows work closely with physicians, nurses, social workers, chaplains, and other healthcare professionals while participating in clinical rotations, scholarly activities, and quality improvement projects. The fellowship prepares graduates to deliver compassionate, evidence-based palliative care and assume leadership roles in hospice and palliative medicine across academic and clinical practice settings.

Eligibility
- Hold an MD or DO degree (or an equivalent medical degree).
- Have successfully completed, or be in the final year of, an ACGME-accredited residency program in one of the following primary specialties:
- Anesthesiology
- Emergency Medicine
- Family Medicine
- Internal Medicine
- Neurology
- Obstetrics & Gynecology
- Pediatrics
- Physical Medicine & Rehabilitation
- Psychiatry
- Radiology
- Surgery (or another ACGME-approved specialty eligible for Hospice and Palliative Medicine fellowship)
- Be eligible for graduate medical education training in the United States.
- Meet all institutional and ACGME eligibility requirements.

Admission Process:
- Apply through the Electronic Residency Application Service (ERAS) during the fellowship application cycle.
- Register with the National Resident Matching Program (NRMP), as the fellowship participates in the NRMP Match.
- Submit a complete ERAS application, including:
- Curriculum Vitae (CV)
- Personal Statement
- Medical school transcript
- USMLE/COMLEX transcripts
- Three letters of recommendation
- Other required supporting documents through ERAS.
- Application Review and Interview. Eligible applicants are reviewed by the fellowship selection committee, and shortlisted candidates are invited for interviews.
- Participate in the NRMP Match. Selected applicants are matched through the NRMP and receive an official fellowship appointment before beginning the one-year training program.
