Bachelor of Physiotherapy (BPT)

• The Candidate should be an Indian National.
• Candidate should have passed HSC examination with science (Bio).
• Candidate Should have cleared NEET Exam.

• Secondary School Certificate(Std. 10th)
• Higher Secondary School Marksheet (Std. 12th)
• Migration Certificate
• Leaving Certificate (LC/TC)
• Entrance Examination Marksheet (NEET/MH-CET/PGP-CET)
• Nationality certificate/ Photo copy of passport/Domicile Certificate/Birth Certificate
• Self attested photocopy of Adhaar Card
• Recent Four Passport Size Photographs
• Medical Fitness Certificate
• Gap Affidavit, Caste Certificate, Caste Validity Certificate
• FIR and Affidavit (In Case Duplicate Marksheet is submitted)
• In case of Name Change attach – Marriage Certificate/Govt. Gazette/Other relevant Document
• For NRI Student: Equivalent Certificate issued by A.I.U. & Grade/Credit point conversion as per Indian Marks Issued by respective Board/University If applicable
