The candidate who have passed B.Sc MLT degree from a recognized University with not less than 50 % of marks in aggregate. An eligibility certificate from the Registrar. Rajiv Gandhi University of Health Sciences, Bangalore, must be submitted by Non Karnataka candidates prior to enrolment.

- Five photo copies of marks card of PUC/+2
- Five Photo copies of SSLC certificate showing the date of birth
- Five photo copies of mark cards in B.Sc MLT
- Five photo copies of physical fitness certificate by registered medical practitioners
- Five copies of passport size and stamp size photographs.
- Five photo copies of Conduct certificate from the institution last attended
- Five photo copies of Transfer Certificate and Migration Certificate.
- Candidates should produce Eligibility Certificate form Rajiv Gandhi University of Health Sciences, Bangalore, (For Non- Karnataka Indian Universities and Foreign Universities).
