Master of Dental Surgery (MDS) in Orthodontics and Dento Facial Orthopedics

A recognized degree of Bachelor of Dental Surgery awarded by a University or institute in India and registered with the State Dental Council and has obtained provisional or permanent registration and has undergone compulsory rotator internship of a year in an approved/recognized dental college.

Documents Required at the time of Admission:

• Allotment letter
• NEET Admit Card of the candidate
• NEET score Card & Data Sheet issued by the CEE, Kerala
• Data Sheet issued by the CEE, Kerala
• Fee remittance slip from CEE, Kerala
• Original BDS Degree Certificate from the concerned University.
• Original Mark sheets of the University Examinations for the whole of BDS course.
• Attempt Certificate issued by the Principal of the College where the candidate studied.
• Certificate from the Head of the institution from where the candidate passed his BDS Degree indicating the DCI approval number and date.
• Completion of Internship Certificate. If a candidate has completed his/her internship from a non-teaching institution, a certificate to prove the recognition of the Centre.
• Original Registration Certificate from the State Dental Council, with latest renewal.
• Documents to prove date of birth.(school record viz. S.S.L.C & +2/ HSE certificate)
• Transfer certificate (T.C) from the institution last studied.
• Conduct Certificate from the College last studied.
• Migration Certificate from the respective University ( If the Candidate qualified from the University other than KUHS ) Eligibility Certificate from KUHS . ( If the Candidate qualified from the University other than KUHS )
• Passport Size Photographs – 5 copies and 1 copy Stamp size Photo ( it should be in white back ground )
