Fellowship in Undersea and Hyperbaric Medicine (FUHM)

1. MD or DO degree from an accredited medical school
2. Completion of an ACGME-accredited residency program
3. Board-eligible or board-certified in the primary specialty
4. Curriculum Vitae and personal statement
5. Letters of recommendation (including residency program director letter)
6. Interest or experience in hyperbaric medicine or wound care
7. Eligibility for medical licensure in Minnesota
8. ECFMG certification for international medical graduates (if applicable)
