Doctor of Pharmacy (Pharm D)

Applicant needs to qualify in (10+2)/intermediate Examination with Science Subject (Both BI.P.C & M.P.C).

The completed application form should be submitted along with photo copies of the following:-

• 10th /SSLC marks certificate
• 10 + 2 / PUC marks certificate
• Transfer Certificate
• Study certificates from 6th to 10th (SSC) and (10+2)/ Intermediate
• Aadhar card
• Migration Certificate
• Seven recent passport size photographs.
