Diploma in Optometry

• (10+2) with PCB/PCM with minimum 40% marks in intermediate exams. (From U.P. Board or any other recognized board.)
• Candidate must have completed age of 17 years of age as on 31st December of admission year. There is no maximum age limit for the admission.

• The admission to this course will be made on the basis of an oral test & merit list prepared on percentages of marks obtained in the qualifying examination. The candidate having higher marks shall be given priority.

Candidates are required to attach the following self attested documents with completely & duly filled application form by the candidate :

• Self attested photocopy of High School marksheet & certificate (5) black & white and 2 colored.
• Self attested photocopy of Intermediate marksheet & certificate (5) black & white and 2 colored.
• Certificate of Caste validity. (5 photocopies)
• Certificate of Father’s Income. (5 photocopies)
• Certificate of Residence (Domicile). (5 photocopies)
• Aadhar Card (5 Photocopies)
• 10 Passport Size Colored Photograph.
• Character Certificate from last institute. (Original)
• 2 Self Address Envelope duly stamped with Rs. 50/- (on each envelope)
• Medical Fitness Certificate bye MBBS Doctor. (Original) – Annexure A1
• Oath Certificate for anti Ragging. (Format Attached) – Annexure – B1 & B2
• Affidavit (Format Attached )- Annexure – C1
• Affidavit given by student to the Principal on Rs. 10/- Non Judicial Stamp paper. (Mentioned on Prospectus)
