Diploma in Medical Laboratory Technology (DMLT)

• Minimum age for admission shall be 17 years of 31st December of the year in which admission is sought.
• A candidate should have passed from recognized board in India in science stream with English.
• Candidates are also eligible from National Institute of Open School (NIOS) recognized by Central Government having Science Subjects and English only.
• Candidate shall be medically fit.
• Candidate shall be admitted once in a year.

• Reservation SC/ST/OBC or any other shall be as per State Govt. rules.

Selection of the 65% candidate should be based on U.P. State Medical Faculty, Lucknow entrance test, and rest of 35% admission can be taken through management quota.

The application should be sent only the prescribed forms supplied along with the Prospectus. This can be had on sending a Demand Draft for Rs. 500/- payable in favor of "Lord Buddha Healthcare Foundation". The application form should be duly filled in by the applicant in his / her own handwriting and forwarded to the Principal, Sumitra Institute of Nursing and Paramedical Sciences, Greater Noida, Gautam Budh Nagar (U.P.).

Attach the required documents in the following order:-

• Acknowledgment Card affixing five rupees stamp.
• Self addressed envelope (24cm x 18cm) with postal stamp.
• Attests copies of following certificates: Original Certificates are to be submitted at the time of admission.
• Secondary school Certificate / Mark Sheet & admit card mentioning date of birth.
• (10+2) Certificate and Mark Sheet.
• Medical Certificate from a Registered Medical Practitioner holding a degree not below M.B.B.S.
• Three recent passport size photograph attested by Principal of School last attended /Gazetted Officer. One photograph to be pasted on the Application form and six extra photographs for office work.
• Migration Certificates.
• Domicile of Uttar Pradesh for ANM candidates.
